Scholarship Program Application Guidelines

Undergraduate Grant Program

Dental Students 2nd, 3rd, and 4th year - $1,000.00

A. Eligibility requirements

1. Financial need

2. Citizenship/permanent resident status

3. (Circle One) 2nd, 3rd, or 4th year dental student or Hygienist Assistant

4. Community Service

B. Process

1. Letter of request for consideration from candidate

2. SAR or GAP Financial Aid Report

3. Two letters of recommendation (one from the dean)

4. Membership in SNDA - verification can be copy of card, letters from
   SNDA Presidents, copies of canceled checks, or receipts from the
   local chapters

5. Reapplication in subsequent years is possible.

Post-Doctoral Programs

Dr. Bessie Elizibeth Delaney - $10,000.00 Female Applicants Only
Dr. Clifton O Dummitt - $10,000.00
Dr. Robert M. Phillips - $10,000.00 Memorial Award

A. Requirements/Eligibility

1. Pursuit of degrees in program of discipline including subspecialty areas of dentistry, public health, administration, research, law, etc.
   
   a. Masters degree beyond length of residence will be considered
   

2. United States citizenship/permanent resident status

3. NDA Membership - must be verified in application

4. Nomination by Program Director Send With Application

5. State cost of program

6. Letters of recommendation (2)

IMPORTANT

Do not mail any items stapled together. If a document has writing on both sides of the page, please send us a duplicate copy so that writing will be on only one side of each page.  

This application must be received in the National Office no later than May 15, of this year. 

NATIONAL DENTAL ASSOCIATION FOUNDATION
SCHOLARSHIP POLICIES AND GUIDELINES  

1. National Dental Association Foundation Staff and their immediate family are not eligible to receive a scholarship from the NDAF Colgate Palmolive Scholarship Program.  

        ("Immediate Family is defined as staff member's spouses, children and grandchildren.")  

2. Members of the Board of Directors of NDAF and Scholarship Committee of NDAF are not allowed to participate in the discussions of awards when a family member is the candidate for scholarship or the family member has put his or her application in for consideration.

3. Mail the form to the NDAF. Send in a complete application to include: Letter of Recommendation, Pictures, References etc. along with this application.
